Job Description




Broad Function
The Health Coach will be stationed at Community Health Posts in the community to engage and support residents in improving their health. He/she will also support the planning and implementation of population health programmes, activities and events.

Key Responsibilities and Duties

  • Engage, motivate and empower residents to lead healthier lifestyles, monitor their health status and delay frailty/chronic diseases;
  • Organise and conduct population health programmes and activities including exercise sessions, health screenings, falls prevention programs, weight management programs, and health workshops/talks;
  • Participate in design, development and implementation of programmes based on local community needs, strengths and assets
  • Be the point-of-contact in the community to provide health coordination, navigation and communication for health initiatives
  • Collaborate with community, social and primary care partners to coordinate care to meet the health needs of residents, including to address social determinants of health.
  • Support documentation and tracking of data for programme monitoring and evaluation
  • Participate in or support organisation/department projects as assigned
  • Any other duty assigned by supervisor
Job Requirements
  • A recognized Degree, preferably in health-related fields like Sports Science, Nutrition, Psychology and Behavioural Science
  • At least 3 years of working experience, preferably in the health and wellness industry
  • Prior frontline experience interacting with patients, residents or customers
  • Health coaching experience will be an advantage but not compulsory as training will be provided
  • Strong interest to work in the community to engage residents in improving their health
  • A strong team-player who is self-motivated and able to work independently
  • Excellent people skills and an upbeat and enthusiastic attitude
  • Excellent interpersonal and communications skills
  • Resourcefulness in problem solving and resilient in overcoming challenges
  • Strong organizational and event management skills, with a keen eye for details
  • Fluent in at least 1 mother tongue language or dialect to communicate with non-English-speaking residents
  • Able to work on weekends and evenings
