Process new applications for \xef\xbb\xbfPeritoneal Dialysis (PD).
Conduct financial Means Test and determine the subsidy quantum for standard cases.
Conduct yearly financial assessment on eligibility for Government subsidies programmes.
Case manage the changes for PD programmes.
Ensure that existing cases are reviewed before the expiry of subsidies.
Liaise with social workers and renal coordinators of restructured hospitals of the patients application status, financial and psychosocial issues.
The first contact with applicants to identify presenting/ underlying issues of applicants.
Refer patients with psychosocial issues or requests to internal and hospital MSW.
Conceptualise and undertake any ad-hoc projects and requests.
To generate reports from different systems to assist with internal and external audit checks.
To prepare reports and statistics for ad-hoc and regular reporting.
Refine the work process for the PD and others beneficiary programmes and liaise with various stakeholders to ensure seamless transfer of care and cost-efficiency.
Document bio-data of patients and requests in case-files for record.
Provide regular and ad-hoc reports on the all the sub-programmes.
Contribute and ensure that the department meet its KPIs.
Requirements:
Degree holder with 2 year experience in social service or healthcare industry
