LentorHealth provides a home medical, home nursing and home personal care service within the home of our Home Care clients. The Home Care Manager will be overall in charge of these three services to ensure smooth operations and functioning of our operations in Home Care.
Location: Macpherson
Responsibilities:
1. Service planning, monitoring and evaluation of the Home Care Service
- Formulate SOPs and service standards for the Home Care Service
- Ensure service quality, safety, and nursing standards for the Home Care service
- Ensure full compliance of Home Care staff to Home Care SOPs, infection control safety requirements and regulations
- Monitor and audit use of supplies to ensure appropriate use
- Ensure clear and timely documentation of patient care service and records
- Liaise with external partners such as locum doctors and external vendors for provision of Home Medical and Home Personal Care Service
- Plan and roster manpower to ensure adequate coverage for provision of services to Home Care clients
- Oversee the Home Care Executive in scheduling of clients\xe2\x80\x99 appointments and manpower rostering to ensure smooth operational provision of services
- Lead and train a team of nurses and care assistants to provide nursing and home personal care to Home Care clients.
- Conduct pre-admission and / or home assessment on potential Home Care clients to ascertain suitability for enrolment into our Home Care Service
- Develop individualized care plan and initiate suitable nursing plans for Home Care clients
- Review and monitor the needs of Home Care Clients, and response to nursing plans
- Coordinate referrals as necessary for Home Care Clients to ensure right-siting of care
- Work closely with multi-disciplinary team to ensure provision of multi-disciplinary care for Home Care clients
- Build and maintain strong relationships with stakeholders such as clients, caregivers, regional health systems and other key partners so as to further the strategic aims of LentorHealth in the home care sector
Requirements
- Diploma or Bachelor Degree in Nursing
- Registered with Singapore Nursing Board
- Minimum 8 years of nursing experience with at least 2 years of home care work experience preferred
- Strong clinical skills
- Strong leadership, interpersonal and communication skills
- Service-oriented with a positive work attitude
- Valid driving license and own a car an added advantage
