Job Description


LentorHealth provides a home medical, home nursing and home personal care service within the home of our Home Care clients. The Home Care Operations Executive will support the Home Care Manager in these three services to ensure smooth operations and functioning of our operations in Home Care.

Location: Macpherson / West Coast Link

Responsibilities

1. Administrative Support for Home Care Service

  • Manage rostering of clients and staff for Home Care Services under the supervision of the Home Care Manager
  • Manage and submit daily revenue report for Home Care Services
  • Manage claims for Home Care staff
  • Maintain necessary records for all Home Care staff including locum staff such as vaccination records where needed.
  • Manage the Home Care equipment and consumables inventory including purchase, stocktaking and allocation of such resources to Home Care staff
2. Home Care Client Support
  • Attend to email and phone inquiries regarding home care services and maintain records and follow up of such inquiries
  • Provide personalized advice on available eldercare services depending on the client\xe2\x80\x99s needs
  • Liaise with stakeholders and facilitate admission into Home Care Service as required
  • Attend to any scheduling issues for Home Care clients
3. Care coordination
  • Coordinate referrals as necessary for Home Care Clients to ensure right-siting of care
  • Work closely with multi-disciplinary team to ensure provision of multi-disciplinary care for Home Care clients
4. Stakeholder relationship building
  • Build and maintain strong relationships with stakeholders such as clients, caregivers, regional health systems and other key partners so as to further the strategic aims of LentorHealth in the home care sector
Requirements
  • Minimum polytechnic diploma, higher education will be an advantage
  • Ability to work with Microsoft Word, Excel and other administrative software
  • Strong administrative and project management skills
  • Good team player with a positive work attitude, strong sense of responsibility and good time management skills
  • Service-oriented with good communication skills
