Job Title: Home Care Nurse

- Perform home assessment using company developed assessment methods and tools.(Necessary training and tools will be provided)- Design tailored care plans for individual clients based on the assessment and following company\xe2\x80\x99s care planning formula.- Facilitate engagement in adherence to the care plan.- Perform Nursing Assessments, act as a general wellness monitor for the client, and communicate any wellness concerns to the client, primary caregiver, or doctor if necessary.- Attend company-provided training and other required job-related training programs dutifully and maintain the necessary level of competency.- Attend to inquiries and contact potential clients to identify their needs and provide information about company\'s services.- Participate and assist in company events, fairs, education talks, and training workshopsJob Requirements:
- Enrolled or Registered Nurse with a minimum of 3 years of relevant nursing experience in general or private hospitals.- Diploma in Nursing or similar, plus licenses to practice nursing in Singapore.- Experienced in-home nursing and/or Dementia Care is preferred but not mandatory.- Excellent observational, leadership and problem-solving skills.- Ability to multi-task, work independently, and be proactive.- Excellent communication and strong interpersonal skills.- Ability to write and communicate in English.- Computer literacy.- Passionate about care at-home, 1-to-1 care.Compensation:
