Lead Care Specialist plays a critical role in providing holistic, patient-centered care to Care Recipient in the community. You will work collaboratively with various Business Units and stakeholders to provide seamless care coordination for Care Recipients and their families to assess needs, develop care plans, and the delivery of healthcare services within the home and community setting. Your focus will be on promoting health and supporting Care Recipients to achieve optimal well-being in their homes and community environments. You will also assist in ensuring best clinical practices and quality patient care are delivered to Care Recipients.

Responsibilities

1. Patient Assessment and Care Planning:

  • Conduct comprehensive assessments of Care Recipients\' physical, emotional, and social needs within the community setting.
  • Develop individualized care plans based on assessment findings, incorporating patient goals, preferences, and available community resources / care services.
  • Collaborate with Care Recipients, families, and healthcare professionals to ensure care plans are person-centered and reflect a holistic approach.
2. Care Delivery and Coordination:
  • Provide direct nursing care to Care Recipients in their homes or community settings, including medication administration, wound care, and monitoring of vital signs.
  • Coordinate and schedule healthcare services, such as home visits, as required.
  • Collaborate with healthcare business partners to ensure coordinated and comprehensive care delivery.
  • Empower Care Recipients to actively participate in their care and make informed decisions about their health.
3. Documentation and Reporting:
  • Maintain accurate and up-to-date documentation of patients\' assessments, care plan, interventions, and outcomes.
  • Adhere to documentation standards and regulatory requirements, ensuring confidentiality and privacy of the patient information.
  • Generate reports and communicate Care Recipient progress, trends, and concerns to Healthcare Business partners or relevant stakeholders.
4. Quality Improvement and Compliance:
  • Comply with professional standards of practice and adhere to policies and guidelines.
  • Participate in audits, evaluate effectiveness of care delivery, and propose quality improvement activities.
Requirements
  • Registration with Singapore Nursing Board (SNB)
  • Bachelor\'s degree qualification in Nursing or higher degree
  • Advanced-Diploma in Nursing (Community Health / Nephro-Uro / Palliative) will be preferred.
  • Certified and valid Basic Cardiac Life Support (BCLS) and AED
  • Minimum 5 years in Acute / Community setting
  • Strong clinical knowledge and understanding of community health practices, including case management and care coordination.
  • Excellent communication, interpersonal, and critical-thinking skills.
  • Empathy, compassion, and ability to establish therapeutic relationships with Care Recipients and their families.
  • Ability to work independently, manage time effectively, and prioritize tasks in dynamic community setting.
  • Proficiency in using healthcare information systems and documentation systems.
