Job Description
:Need 2 headcounts for 5 months contract, convertible to permanent headcount.Responsibilities and Duties:
- Perform social and financial assessment of patients and make recommendations.
- Provide counselling to patients and their families and lead in family conferences.
- Liaise with relevant organisations for collaboration and networking in care of the elderly.
- Attend to the social needs of destitute patients.
- Participate in a multi-disciplinary team on patient-care.
- Conduct groupwork with the elderly patients or their caregivers.
- Implement initiatives and support programmes that enhances the care and support of elderly patients and their caregivers within the community
- Monday to Thursday : 8:30am to 5:45pm
- Friday : 8:30am to 5:30pm
- One Saturday per 4 weeks: 8.30am to 12.30pm
- Total: 42 hours per week
