In addition, they also provide health screening programmes and health promotion advisory services. What we are looking for: Anyone with a passion for a nursing position. Someone with patient care skills and good communications skills with the patient. Attention to detail good in customer service and positive attitude. Responsibilities: Provide nurse counselling and patient education on chronic disease management and health promotion. Able to assess patients' nutritional needs and provide basic nutrition education. To teach specific tests for monitoring of chronic diseases e.g. blood glucose, blood pressure etc. Tracking of patient's progress in chronic disease management and lifestyle changes. This can involve face to face consult, teleconsult and telehealth monitoring. Case management of complex chronic cases may involve home visits to assess the patient's home environment, family support, mobility and social status. Conduct Diabetic Foot Screening and Foot Care education. Referrals of 'At-Risk' cases to the Podiatrist based on PCN's protocol and in consultation with the doctor. Work closely with the Primary Care Coordinators (PCC) in care condition and care transition. This will involve liaising with the public institutions, GP clinic team, community health and social providers. Participate in PCN meetings, CME activities and case discussions. When needed, function as a member of the frontline clinical workforce and support the doctors in various clinical activities. Any other duties as delegated by the Medical Director or Director Healthcare Services. Requirements: Diploma/Degree in Nursing or equivalent with at least 5 years of clinical nursing experience. Registered with Singapore Nursing Board. Self-motivated and able to work well in a team. Good interpersonal and communication skills Proficient with Microsoft Office Applications.
