The Manager is responsible for the environmental scanning, development, execution and monitoring of engagement and communications strategy to build strong and sustainable relationships between NHG entities and collective ecosystem of community health and social care providers, agencies, organisations and primary care providers. This will be the foundation of NHG\'s role as the regional health manager to join up care, forge place-based partnerships to anchor health and social care in the community for the 1.5 million residents in Central and North Singapore.He/She is responsible for developing, enhancing and marketing of digital enablers (eg Customer Relationship Management (CRM), Partners Portal, NHG Cares App, Resident Wallet, etc) to support NHG\'s collaborative partnerships model for population health, with partners and residents.He/She is responsible for formulating and managing contracts and workplans with partners, to drive transformational, sustainable clinical and service integration, anchored by trust, shared values and goals.He/She is responsible for collaborating with the Membership team to strategise and execute NHG\'s engagement plans for the national enrolment campaign.Major Duties and Responsibilities
