About The Role Job Responsibilities: 1. Support elderly patients and their caregivers with: - Psychosocial and financial support - Casework and counseling services 2. To facilitate successful integration back to the community by: - Collaborating closely with the multi-disciplinary team - Implementing a comprehensive and coordinated care plan - Implementing initiatives and support programmes that enhances the care and support of elderly patients and their caregivers within the community Candidate Requirements Technical Competencies: -Ability to communicate with vernacular languages speaking clients. (in order to provide psychosocial, financial support and assistance to Vernacular Languages Speaking Clients.) Core Competencies: - Degree and postgraduate diploma in Social Work. Preferred achievements / characteristics: - Must have an interest to work with the elderly in a healthcare setting Only short-listed candidates will be contacted for an interview.
