- CALLING ALL NURSES FROM INPATIENT WARDS WANNA MOVE OUT OF WARD SETTING
- WORK 3 DAYS PER WEEK
- CAN REFER FRIENDS TO WORK TOGETHER
Note:
Criteria : Strong inpatient wards experience (without palliative exp is fine)
Working hours : Office Hours, 5 Days 8.30-6.00pm
Remark : Each nurse will have 2 days on call per week (For example, nurse will work til 6pm, and on call starts from 6.30pm to 8am - they have their off day next. So essentially, they only work 3 days per week)
Job Scope :
- This role will be abit more like case management - manage the care plan for the patients
- You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
- You will be part of the most dedicated, compassionate and caring teams comprising doctors, nurses and medical social workers providing an all-in-one professional care to patients at home at all hours, seven days a week
