Pharmacist
Job Description
We're looking for a passionate pharmacist to educate andempower our patients. As part of the Yishun Community Hospital (YCH) Pharmacyteam, you'll help us show patients how YCH provides seamless care through thecontinuum of care and integrates them back into the community.You'll be providing patient-centered care throughinterviewing and engaging our patients and their caregivers during medicationreconciliation. You'll set pharmacotherapy goals for individual patients. Forselected groups of rehabilitative patients, you'll assess their abilities toself-administer their medications during their stay in YCH. You'll provide allpatients with their personalised Patient Medication List and use the list toeducate and empower them and/or their caregivers on the names, indications andcommon side effects of their medications.You'll be given opportunities to conduct educationalsessions with our patients and other healthcare professionals onmedication-related topics. You'll be able to carry out research studies, runambulatory clinics and review medications at the nursing homes.Requirements; Bachelorof Science degree in Pharmacy from an accredited University recognized by the Singapore Pharmacy Council Registeredwith the Singapore Pharmacy Council Valid practicing certificate
