Perform medications reconciliation & review, and ensure the appropriateness of patient drug therapy
Dispense medications and give appropriate medications counselling to the patients & caregivers
Provide timely and accurate drug information to healthcare professionals and patients / caregivers
Involve in the education and training of pharmacy staff and other healthcare professionals
Participate in projects/activities which support the goals of the department and hospital
Requirements:
Degree in Pharmacy
Registered Pharmacist with the Singapore Pharmacy Council
At least 2 years of experience and demonstrated competency, or with relevant post graduate qualifications
Have experience in clinical patient care practice
Strong analytical skills
Excellent interpersonal skills in working with both internal and external stakeholders
