I-CARE PCN PTE. LTD. is the headquarters of a network of GP clinics aimed at delivering better and more holistic chronic care to patients in the community via a team-based approach. Our GP clinics are supported by a team of nurse counsellors and care coordinators thus freeing doctors up to focus on patients\' medical issues. We conduct mobile health screening services and nurse counselling for patients with Diabetes, Hypertension, Lipid Disorders, Asthma and Chronic Obstructive Pulmonary Disease (COPD) island-wide.ob Description
- Coordination of healthcare services
- Follow up with patients on appointments to ensure continuity of care
- Handling payment collection, recordkeeping, verification, consolidation and analysis of patient\xe2\x80\x99s clinical data
- Liaise with clinics to improve care for chronic patients
- Assist in other duties when required
- Candidate must possess at least a diploma in Healthcare Administration, Pharmaceutical Science, Food Science or related field.
- Fluency in English, Chinese and a local dialect (Hokkien, Teochew, Cantonese)
- Must be patient-oriented, mature, independent yet able to work as part of a multi-disciplinary team and have good interpersonal and communication skills.
- Positive and Proactive Team player
- Proficient in MS Office applications (Word, Powerpoint, Excel), Macros and Visual Basic knowledge will be an added advantage
