Part of a multi-disciplinary team to support activities in accordance with curriculum objectives, developmentally appropriate practice and program philosophy
Introduce, implement rehabilitative therapies and/or programmes. Review and evaluate therapies and programmes to see if they are relevant to residents.
Facilitate, coordinate and plan in-house programmes for residents. Monitor residents involvement for in-house activities and update individual residents care plan (ICP) report to respective case or social workers/managers
Work closely with the Programme team to ensure programmes rostered to do are delivered
Plan, develop program materials, design and implement group activities
Work with key workers to implement strategies for behaviour management
Responsible for progress monitoring of achievement of residents care plan goals
Community Partnership and Volunteer Management
Source, develop and sustain collaborations with other social service agencies, community stakeholders, corporate partners and, government agencies where necessary for effective programme and volunteer management
Work with volunteers to deliver volunteer led programmes
Responsible for maintaining the volunteer database
Qualifications:
Preferably Diploma in related field
Those without diploma will be considered based on working experience
Good communication, teamwork and leadership skills
Ability to work independently and meet assigned deadlines
Knowledge of Microsoft Office 365 applications (Outlook, Word, Excel, PowerPoint)
Work location: Eunos
Work hours: 5 working days, including one weekend
