Job Description

Broad Function
The Obesity & Metabolic Service (OMS) is a patient-centric service which uses an inter-disciplinary team approach to manage obesity and obesity-related complications in a one-stop clinic. The role of the coordinator is to support the data collation and analysis for key programme indicators, and ensure continuity of care for patients across inpatient, outpatient and primary care settings.
Key Roles & Responsibilities
Specific
i. Coordinating patient care and tracking of patient journey and outcomes to ensure continuity and effectiveness of care. This includes:
Coordinating of patient clinic appointments across a several disciplines which require close follow-up due to the complex nature of the condition and post-operative follow-ups to optimise patient outcomes
Coordinating workshops for patients (e.g., dietetics, psychology, including Obesity Community Programme)
Involved in health coaching coordinating care for patients (e.g., follow-up)
Liaising with external parties for up- and down-stream care, including programmes with primary care, ActiveSG
Triaging of incoming referrals
ii. Preparing annual report on progress and outcomes of programmes for Clinical Board, Quality, Productivity, and Improvement Committee (QPIC), and annual clinical performance review.
iii. Collating and transcribing clinical measures into a database for programme and clinical performance evaluation.
iv. Analysing programme evaluation questionnaires (non-clinical).
v. Conceptualise development, implementation, evaluation and continuous improvement workflows in collaboration with healthcare team to improve clinical service and care.
vi. Provide secretariat and administrative support as assigned by supervisor(s). This may include minutes taking, preparing meeting slides, tracking matters arising, keeping records, liaising with clinicians and SOC operations team on clinic templates etc.
General
i. Where directed by Reporting Officer, and/or Deputy Director / Director, to support in other Woodlands Health-related projects and tasks.
Job Requirements

• Bachelor's degree in in science or health related field
• Minimum of 2-3 years in programme coordination or a related field
• At least 2 years and 5 years of clinical / research experience in healthcare sector for Degree and Diploma holder, respectively.
• Not averse to review and manage diabetic foot wounds.
• Experience in supporting write-up of clinical trial protocols and/or DSRB applications is an advantage.
• Proficient in Microsoft Office applications (Microsoft Word, Excel, PowerPoint) is essential.
• Familiarity with healthcare systems, research statistics programs such as SPSS or GraphPad Prism and research registry systems such as REDCap will be an advantage.
