Job Description
Responsibilities
- Co-ordinate the daily registration of clients at the Senior Care Centre (SCC).
- To ensure SCC staff conduct group therapy sessions for the clients under the instruction and guidance of Physiotherapist and Occupational Therapist.
- Organise and coordinate family conference to assist them in understanding of the physical, social and mental condition of the client.
- Participate in client care meeting/inter-professional meeting at SCC.
- Report to the Director of Nursing and work with physiotherapist, occupational therapist, clients & their families, physicians and other nursing staff.
- Update clients\' service attendance records daily; keep daily and monthly clients\' census.
- Monitor, evaluate and record client progress according to treatment and care plan.
- To liaise and coordinate with transport provider on booking of weekly transport for SCC clients.
- Receive and liaise with AIC referral team regarding referrals.
- At least 2 year experience working as Supervisor in Senior Care Center/Day care/Senior Activity Centre.
- Background of admin/social work preferred.
- Able to do family counseling and admission.
- Proficient to handle IRMS, fee charges, familiar with AIC/MOH policies.
