What the role is:You will be part of a dynamic team that drives and coordinates the service and capacity planning, and national implementation of home-based care services. You will be working in a fast-paced and dynamic environment that would require the ability to manage multiple priorities and stakeholders at the same time.What you will be working on:In this role, you will be involved in the policymaking, planning and implementation of home-based care services. This will involve: \xe2\x80\xa2 Reviewing, developing and implementing policies and services to transform home-based care, so as to allow our growing population of seniors to receive seamless holistic care and age-in-place in the community, and avoid institutionalisation.
