Job Description

Job Purpose

1. Specialist to lead and conduct investigations on significant workplace incidents and near misses in SMRT centrally.
2. Specialist to advise Business Units (BUs) in matters relating to investigation standards, methodologies, root cause analysis.
3. Sustain investigation officers community of practice with TSIB and within SMRT.
4.Conduct analysis of past incidents to ascertain trends of concerns and suggest areas for systemic improvement.
5. Involve in BUs independent audit programme to validate ground compliances and determine the effectiveness of controls within the Risks and HSE Management System in meeting organization objectives; recommend initiatives for continual improvement.
6. Report outcomes of investigations and audit to Management & Board.
7. Represents SMRT and be the liaison and coordinates with regulators and external agencies (eg MOM, TSIB, LTA) for the purpose of incident investigations.

Responsibilities

Duties of SM Safety Investigation & Audit are listed below. Note that the list is not exhaustive and related duties and responsibilities may be assigned from time to time.

1. Ensure all BUs adopt the approved investigation methodology and adhere to the investigation directive in SMRT.
2. Ensure a high-quality standard of all investigation reports from BUs, and that all incidents are able to establish the root causes and appropriate follow up recommendations.
3. Maintain the registry of all investigation reports in SMRT
4. Maintain the tracking system to ensure competency of all identified Investigation Officers in SMRT
5. Perform Investigation as directed by Board, GCEO and/or CSSO, where required
6. Prepare, issue and file investigation reports and supporting documents
7. Involvement in risk-based audit and inspection program with focus on safety and security matters of the employees, stakeholders, as well as system within the respective business units in SMRT
8. Validate ground compliance to established authorized operating document, past incident/ audit/ inspection findings and risk registers.
9. Trend and provide regular updates to management on outcomes investigations based on the scope or area assigned to him/ her.
10. Track and monitor corrective actions arising investigations.
11. Main SMRT representative with relevant regulators and authorities to coordinate on incident investigations.
12. Support Head Safety Investigation & Audit to provide regular investigation trend status to safety meetings (e.g. Management Committee and Board Safety & Security Committee Meetings.)

Qualifications & Work Experience

Qualifications and work experience include:

1. BSc degree in Engineering or Science (Environmental, Occupational Safety and Health Occupational Safety and Health)
2. Experience in Land Transport, Oil & Gas or Aviation Sector Incident Investigation Works is highly advantageous.
3. Knowledge and experience in rail operations and maintenance is highly advantageous.
4. Trained and experienced in investigation methodologies such as 5M or Systemic Cause Analysis Technique, with at least 10 years of investigation experience preferred
5. Registered Workplace Safety & Health Officer (WSHO) and Fire Safety Manager (FSM)
6. Qualified IRCA lead ISO45001 /ISO14001/ISO9001 auditor, with at least 5 years of working experience in conducting internal audits

Skills

Technical skills include:
1. Good knowledge on Workplace Safety and Health (WSH)
2. Proficient in MS Word, Excel and PowerPoint

Generic skills include:
1. Independent, and able to multitask to execute tasks according to deadlines
2. Critical thinking and problem-solving skills
3. Excellent investigation report writing skills
4. Great interpersonal and communication skills
5. Good time management
