Support the HOD to lead the team of programme managers in the service development and implementation of community mental health (CMH) programmes to support people at risk/with mental health conditions and their caregivers in the community.
Guide team in the service development and implementation of CMH programmes under the Community Mental Health Masterplan
Support to establish a quality framework for continuous review and enhancement of the care models to meet the intended outcomes and support the needs of the clients and caregivers
Guide the team on the continuous review of the service and care models of the community mental health programmes to be able to meet the needs of clients and caregivers including the analysis of submitted data to support programme evaluation.
Monitor programme effectiveness through a systematic evaluation framework developed in partnership with internal stakeholders.
Coordinate collation and review of data, performance indicators of CMH programmes to distill key learning points and recommendations.
Share findings with internal and external stakeholders where relevant.
Review & streamline work processes for data submission.
Develop guide and structural support to the team on raw data cleaning & analytics.
Identify good practices for cross sharing amongst programme managers and service partners at appropriate platforms.
Support the HOD in identifying learning needs of service partners and working with external and internal stakeholders to tailor training programmes to support service providers to support their clients/caregivers and meet emerging needs.
Work with the team to identify learning needs and gaps of CMH partners through survey/engagement, and consolidate the findings for analysis.
Guide the team to co-develop systematic training programmes for CMH partners based on the mental health and dementia competency frameworks.
