: Job Responsibilities: * Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members. * Perform referrals to other service providers and conducts home v : Job Responsibilities: Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members. Perform referrals to other service providers and conducts home visit where necessary. Work closely with multi-disciplinary team members. Support projects and programmes. Attend training and case work supervision. Job Requirements: Degree in Social Work or a Graduate Diploma in Social Work. Registered with Singapore Association of Social Work. Experience in means testing and healthcare setting is advantageous .
