Job Description


:

The Senior Program Associate will lead a small team of staff and volunteers and be responsible for the daily operations of the Active Ageing Care Hub (AACH)\xe2\x80\x99s programmes and activities, fulfilling the objectives of the AACH and meeting the targets set by funders and management. The key role of the AACH is to ensure seniors within the service cluster are engaged and supported through:

  • Outreach to seniors within the service cluster and profile them according to their needs
  • Active ageing programmes according to the needs of the active seniors
  • Befriending service for isolated & vulnerable seniors
  • Information and referral services
Planning and operations
  • Establish a collaborative relationship with the Day Care team and work with them on shared space and visitors\xe2\x80\x99 management.
  • Develop and implement outreach plans to engage and profile seniors for appropriate follow-up actions
  • Plan, implement and manage active ageing programs and activities for seniors which are age-friendly and culturally appropriate, and within budget.
  • Develop the network and collaborate with community partners and national bodies, for example, grassroots, social service agencies, NAC, SportSG, etc. on suitable programmes and activities for seniors
  • Empower and develop active seniors to serve other vulnerable seniors in the community.
  • Establish referral process with care providers and refer seniors to them if required and coordinate support services for the seniors.
  • Collect regular feedback from befrienders on vulnerable seniors\xe2\x80\x99 conditions and engage appropriate community partners to address those issues
  • Attend networking sessions and management meetings as required
  • Plan marketing activities and develop publicity materials with the support of a Corp Communication representative.
Administrative work
  • Attend to all administration required for the AACH, including volunteer data management, and report submissions in a prompt and timely fashion
  • Support necessary evaluation and all critical reporting processes for the management and funders of the programme
  • Handle enquiries about the work of AACH
Supervisory role
  • Work with internal and external partners in recruiting and training staff and volunteers to assist with the work of AACH
  • Engage, supervise and retain staff and volunteers
Any other duties assigned by the management.

Qualifications:
  • Diploma in Social Service preferred with 3 years of social service, community engagement or eldercare experience.
  • Some management/start-up experience preferred
  • Proficiency in digital skills (e.g. google space and CRM)
  • Pleasant, resourceful, and proactive
  • Good collaborative and stakeholder management skills
  • Good problem-solving and planning skills
  • Good interpersonal and people management skills
  • Willing to outreach and engage seniors in the community e.g. seniors\xe2\x80\x99 own homes, in the park, or coffee shops
Additional Details:
  • Alternate Saturday work
  • Prepared to work on weekday nights and weekends for events and activities when necessary
