The Division of Community Geriatrics in Department of Geriatric Medicine cares for patients in the community and home care settings, as part of post-acute care follow up or hospital-at-home care.

As a Resident Physician / Senior Resident Physician in Community Geriatrics, you will be part of a multi-disciplinary team providing medical care for patients in their homes. Doctors who have interest in caring for older people with acute medical issues as well as geriatric related problems such as delirium, dementia, falls, frailty & end-of-life care will be advantageous. You will be responsible for the management of patients under the supervision of senior doctors and in collaboration with other healthcare professionals. Your responsibilities include providing medical consultations and treatment of acute and chronic illnesses, addressing the needs of home-care patients and their families, who will include a comprehensive geriatric assessment of their physical, cognitive, social and functional domains of their lives, and other duties as assigned by your supervisor.

Job Requirements:

• Possess a basic or postgraduate medical qualification that is recognized by the Singapore Medical Council;
• Conditionally or fully registered under Singapore Medical Council;
• Registered as a medical doctor in the country of conferment of medical qualification and last place(s) of practice;
• Possess at least 3 years of post-graduate working experience as a medical officer (or equivalent);
• At least 6 months of working experience as a medical officer (or equivalent) in a general medicine or geriatric department in an acute hospital;
• Active in clinical practice for at least the past 1 year
• Provide evidence of English language proficiency and meet the acceptable score specified by the Singapore Medical Council if the candidate's basic medical qualification is not conducted in English Language.
• Experience in working in home care or community setting is not essential but will be an advantage
