As a regional provider for integrated healthcare servcies in Singapore, Malaysia and China, the Group incorporates a chain of medicare centres and nursing homes, hospital services, a careskill training centre, a chain of senior services centres, home care services, ambulance services, physiotherapy services and traditional Chinese medicine services. The Group also provides consultancy for the development and management of retirement living communities.

1. Act as a coordinator for the admission of residents under Ministry of Health (MOH) Portable Subsidy. 2. Perform financial counselling for all residents under MOH\xe2\x80\x99s Portable Subsidy, including the preparation of estimated bill size, admission contract and other administrative matters. 3. To perform and monitor National Means Testing (NMTS) for portable subsidy residents and ensure the resident NMTS are valid and updated according to MOH\xe2\x80\x99s requirement. 4. Refer referrals and new admissions assigned by AIC for all ECON Medicare Centres and Nursing Homes 5. Monitor and track the available licensed beds for portable subsidy residents all ECON Medicare Centres and Nursing Homes. 6. Act as a resource person for the MOH ILTC portal and IRMs for all ECON Medicare Centres and Nursing Homes. 7. Raise referrals and applications for Nursing Home subsidy for ECON private resident. 8. Assist Home and Centre in putting up the following applications for their residents

• Shelter Home
• Day Care
• Day Rehab
• Home Modification
• Senior Mobility Fund
• Meals on Wheelsg.
• Any other Home and Centre Based schemes available

9. Supervise and check all applications for government schemes prior to submission. Applications would include the following, and other schemes that may be available.

• Interim Disability Assistance Programme for the Elderly (IDAPE)
• Eldershield Claim
• Medical Fee Exemption Card (MFEC)
• Medifund Claime. Elderfund
• Careshield Life / Medishield Life
• Home Caregiver Grant
• PGDAS
• Silver support schemes encashment
• Any other financial schemes available

10. Supervise in the monthly tracking of Subsidy Change report for billing and accuracy forall ECON Nursing Homes and Medicare Centers 11. Assess and put-up applications for medifund claims quarterly for all ECON NursingHomes and Medicare Centers. 12. Assist the Social worker or supervisor in engaging with clients for psychosocial needs,conflict management and family conference. 13. Perform such other duties and responsibilities that may be assigned from time to time
Requirements 1. Diploma in Social services / Nitec in Community or Social Services or any equivalent qualification with at least 3-year relevant working experience 2. Good interpersonal and communication skills 3. Team player 4. Service oriented and customer-focused 5. Competent in IT skills
