About The Role Conduct home visit and assessment of client's needs and financial situation Prepare social report and present to Senior Social Worker for approval Develop care plans and enroll clients for elderly service program Provide practical s About The Role Conduct home visit and assessment of client's needs and financial situation Prepare social report and present to Senior Social Worker for approval Develop care plans and enroll clients for elderly service program Provide practical support to operational needs of department Prepare, file and maintain proper documentation and updates of clients' data Regular follow up and review to assess progress and additional needs of clients Engage and coordinate with different stakeholders to support delivery of care plans for clients Candidate Requirements Technical Competencies: Diploma holder in any discipline Experience in social service sector will be an advantage Computer literacy in Microsoft office Able to speak English, Mandarin and dialects Core Competencies: Good writing, communication, and interpersonal skills Team player with integrity, patience and commitment Preferred achievements / characteristics: Only short-listed candidates will be contacted for an interview.
