Job Description

Co-work with social worker on cases and assist them with their daily tasks. Assessing clients and determining the type of help they need.
Working with social workers to determine Intervention plans and termination of cases.
Assist in fieldwork and home visits
Researching and coordinating community services
Helping clients to complete the necessary paperwork to apply for assistance, administering various subsidies and claims
Undertaking regular check-ins with clients.
Documenting client behavior, maintaining of case updates/recordings, filing assessment, minutes taking
Provide support in any other work duties or projects (if any)

Job Specifications

Possess a Diploma in Social Work, preferably with 2 years of relevant experience Experience in working with special needs children/adults would be an added advantage Fresh graduate may be considered Pro-active work attitude with good interpersonal, communication and organisational skills A team player who is able to work effectively and harmoniously within a multidisciplinary team setting Able to work independently and effectively under pressure * Proficient in MS Office
