The Social Work Coordinator will assist in providing psycho-social assessment and interventions, delivery of programmes and performing community development work, in order to support clients in coping with their circumstances and to facilitate resolution of issues.
Duties and Responsibilities
Conduct financial counselling and assessment, and ensure timely application and/or renewal of means testing and financial assistance
Support care enquiry, case assessment, care planning, goal setting, interventions, and closure of cases
Support development, planning, preparation, implementation, and review of programmes
Establish good rapport and build professional relationships with clients and significant others (e.g., family, caregivers, etc)
Participate in case discussion with the care team
Support in networking with stakeholders for resources to support beneficiaries
Conduct data collection
Perform good documentation, maintenance of case files, and preparation of reports, according to the required documentation standard
To perform any other administrative duties as required
Job Requirements
Diploma in Social Work, Counselling or Community Services
Preferably 1 to 2 years of relevant experience in healthcare or community care sector
A team player with good interpersonal and communication skills
Passionate in working with the elderly and persons with disabilities, and strongly believes in advocating for quality of life through a holistic approach
Displays patience, empathy, and strong listening skills
Able to converse in English and at least one other local language
Candidate must be willing to commute between our 2 centres (Lengkok Bahru and Jurong Point Shopping Centre). MRT accessible.
