Conduct assessment, planning and counselling intervention for cases.
Conduct case conferences and consultations with case workers and partners.
Participate in networking and case management meetings when necessary.
Document and evaluate case outcomes.
Develop and review appropriate counselling intervention strategies.
Ensure reports on cases are submitted in a timely manner.
Information and Triage
Refer clients to appropriate agencies for further intervention when necessary.
Part of the Intake & Triage Team to conduct intake assessments for new intakes based on the practice standards of the Code of Social Work Practice and Practice Guide

Requirements:
At least1 year of relevant clinical experiences
At least 1 year of relevant experience in a Social Service Sector
Competent in therapeutic modality to counsel individuals and couples
Competent in conducting workshops /talks related to mental health.
Has a passion to advocate for mental health through conducting talks and workshops
Possess excellent inter-personal qualities and strong work ethic
High level of interpersonal, written and verbal communication skills
