Provides leadership and oversees Community team & volunteer befrienders to ensure service delivery according to programme objective in accordance to ethical and legal guidelines.
Conducts comprehensive client assessments to collect functional, environmental, bio- psycho-social, financial, employment, housing, and educational information as appropriate to develop a comprehensive case plan. The Social Worker maintains professional relationships with service users, acting as a guide, a support, an advocator and intermediary.
Key Responsibility Areas:
Oversee planning, care coordination and referral processes.
Liaise with relevant community partners to administrate financial assistance schemes.
Provision of supervision and direction for community executive team & volunteer befrienders to ensure care coordination, service provision and service linkage for clients and caregivers.
Oversee training and development of community executive team & volunteer befrienders with relevant competencies, team progress and performance in accordance with service provision.
Oversee planning with staffs, implementation and monitoring of care plans, referral and follow-up processes.
Identify and work with relevant stakeholders and community partners to support care plan.
System development and linkage to build network with stakeholders, community partners and agencies to facilitate care support for clients and caregivers.
Establish and review referral, workflow and communication processes, development and update of service manual.
Provide psychoeducation and care planning for clients and caregivers after diagnosis from primary care.
Participate in multidisciplinary team meetings;
Maintain accurate records, update records and prepare reports for reporting to governing agencies and government mean-testing application;
Participate in internal and external training, supervision and team
Any duties as assigned by the Case Manager and the Executive Director.
Experience:
Minimum Degree or Post Graduate Qualifications in Social Work from recognised institutions
Minimum 2 years of experience working with families and seniors preferred
Qualifications: Minimum Degree or Post Graduate Qualifications in Social Work from recognised institutions
Other Requirement:
Required travelling to different locations
Competencies:
Knowledge of Person-centric care, dementia, mental illness and disease management.
Team player with strong interpersonal and communication skills.
Preferably bilingual and knowledge of Malay and Chinese dialects will be an added advantage
