To effectively manage cases receiving foster care services and to support programmes/events of Fostering Department and the Agency. Job Responsibilities

• Manage cases receiving foster care services and support programmes or events of Fostering Department and the Agency
• Conduct in-depth assessment and prepare Foster Parent Assessment Report (FAR), and present findings at the CIC Approving Panel and follow up on decisions and recommendations
• Conduct regular CANs assessments to assess how the foster child is doing as well as the foster parents’ capability in meeting the needs of the child
• Conduct necessary statutory visits to ensure that the placement is able to safeguard and promote safety, stability and wellbeing
• Organize and participate in inter-agency efforts to support the welfare and development of the foster child or CYP
• Provide a 24-hour hotline for foster parents or kinship care per duty schedule and follow up on outcomes
• Support foster placement to prevent breakdowns in existing care arrangements for the foster child
• Participate actively in case movement through regular team meetings and monthly case review sessions
• Identify at-risk situations and develop strategies and intervention plan to address child safety issues and wellbeing
• Monitor and evaluate the effectiveness of case management and other programme and activities extended to foster or kinship care families
• Submit accurate operation reports, case reviews, discussions to BT and other governing bodies

• Bachelor of Social Work or Graduate Diploma in Social Work or Master of Social Work
• Registered Social Worker with SASW preferred
• Experience in working with children or CYP and their families
