Job Description


1. Reporting Authority To report to the Head of Home (HOH) 2. Job Functions \xc2\xb7 Ensure that residents comply with the Centre\xe2\x80\x99s Rules and Regulations. \xc2\xb7 Setting up of Individual Care Plan for all residents \xc2\xb7 Responsible for the Clinical Intervention components in the Centre (include counselling and caring services). \xc2\xb7 Assist in ensuring safe custody of residents \xc2\xb7 Work closely with other departments/government agencies/Healthcare institutions \xc2\xb7 Ensure that the security measures are adequate in the Centre. \xc2\xb7 Assist Deputy Head of Home in submission of reports of all serious incidents and occurrences of unusual and serious nature pertaining to staff and residents to the Home\xe2\x80\x99s Management Committee. \xc2\xb7 Work in close liaison with law enforcement, welfare regulators and other related agencies. \xc2\xb7 Assess potential clients prior to admission to ensure suitability. \xc2\xb7 Work closely with other departments to ensure residents\xe2\x80\x99 suitability in the home and to assess and discharge when needed. \xc2\xb7 Prepare reports for submission timely and submit for vetting by the Deputy Head of Home. \xc2\xb7 Ensure that all records, accounts, documents and correspondence are properly maintained, kept current and up-to-date. \xc2\xb7 Maintain casefiles of all residents and ensure casefiles are updated. \xc2\xb7 Facilitate applications and renewals of Medical Fee Exemption Cards for all residents \xc2\xb7 Facilitate applications and renewals of Means testing for all residents \xc2\xb7 Facilitate group counselling, family sessions, one-to-one sessions and maintain case recordings. \xc2\xb7 Facilitate home/hospital visits and other significant tasks \xc2\xb7 Work closely with the residents and their family for possible reintegration \xc2\xb7 To present cases to the staff of latest updates \xc2\xb7 Update residents\xe2\x80\x99 family of significant incidents/hospitalisation/happenings \xc2\xb7 Responsible for the development of the Centre \xc2\xb7 Provide support to the Operations of the centre \xc2\xb7 Carry out any other tasks assigned by the Head of the Home, Licensee and Governing Board \xc2\xb7 Responsible to lodge a Police report when necessary. 3. Candidate Requirements \xc2\xb7 Diploma or Degree in Social Work / Psychology/ Counselling \xc2\xb7 At least 1 to 2 years of casework and counselling experience \xc2\xb7 Good care management and counselling skills \xc2\xb7 Certified in doing mandatory counselling cases will be advantages \xc2\xb7 Good interpersonal and communication skills \xc2\xb7 Meticulous and a team player \xc2\xb7 Strong attention to detail and has the ability to work efficiently, accurately, as well as to follow up on pending items in a timely manner Job Types: Full-time, Permanent Salary: $2,800.00 - $3,200.00 per month Benefits:
