The Salvation Army, Children and Youth Group- Gracehaven is a residential home for children and youths who require care, protection and rehabilitation due to risk behaviours and a lack of parental supervision. We aim to help children and youths reintegrate back into their family and society through an array of holistic programmes and activities specially designed to cater to the well being of each individual person. At Gracehaven, a team of professionally trained social workers work towards building holistic and individualized care plans that are tailored to meet each person\xe2\x80\x99s needs. DESIGNATION : Social Worker (IPAC)

RESPONSIBILITIES
In addition to caring for children and youths, Children and Youth Group - Gracehaven also runs an Interim Placement and Assessment Centre (IPAC) in the residential home. This temporary care environment will allow children under child protection to be in a safe place while assessment into their needs are undertaken to facilitate their placement in the most appropriate care environment. As a Social Woker in IPAC, you are expected to maintain a caseload as assigned by senior staff members and provide individual casework, group and family work intervention action plans including behavioural management of clients in care.
Key Responsibilities

• Participate in social work practice across disciplines
• Build rapport and professional relationships with clients and significant others
• Conduct needs and intake assessments
• Perform case and care planning and goal setting
• Implement case and care plans and interventions
• Review casework interventions for improvements
• Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities
• Maintain case notes and documents
• Prepare social work reports
• Conduct environmental scans and community need assessments
• Collaborate with other agencies to pool community resources
• Support development of community development initiatives
• Implement community development initiatives
• Review community development initiatives for improvements
• Perform the development and planning of programmes to address service gaps
• Implement programmes
• Support evaluation of programmes
• Develop own self-care plans
• Guide volunteers and placement students in their work
• Support the implementation of standards of care, organisational health policies, procedures and programmes
• Identify areas of improvement in models of care and service delivery based on evidence-based practices
• Respond to crisis situations based on organisational standard operating procedures

QUALIFICATIONS

• Bachelor Degree/Graduate Diploma in Social Work is a mandatory

Preferred Candidate Attributes

• Member of Singapore Association of Social Workers is preferred
