Responsibilities

• Plan and create/co-create residents\xe2\x80\x99 reintegration plan 4-6 months in advance with input from Senior Social Worker/Counsellor
• Engage residents to prepare them for meaningful transition from residential care to family/kinship care through individual and/or small group sessions
• Drive implementation of aftercare plan upon resident\xe2\x80\x99s discharge and manage risk
• Assess, implement and review residents\xe2\x80\x99 reintegration process to family/community
• Interview, assess and review resident and family\xe2\x80\x99s development/progress
• Address and monitor areas for improvement
• Engage, communicate and advocate with community agencies and/or external parties for the provision of housing, financial, educational/employment and/or treatment services
• Adhere to documentation practices required of the role and in accordance with Standards of Care for Homes
• Adhere to ethics of the profession

• Degree in Social Work / Counselling preferred
• Certified medically fit for the job
• Basic knowledge of trauma and trauma-informed care principles
• A creative, dynamic and proactive team player
• Passionate about working with youths
• Good written and oral communication, especially English
• Possesses good Interpersonal skills
• Possesses good boundary management
• Possesses good listening and communication skills
• Good knowledge of Microsoft Office (Word, Excel, PowerPoint)
• Willing to be supervised
• At least 1year of relevant experience in related profession or case management
• Able to multi-task and work independently
• Comfortable to do fieldwork

• 5-day work week
• To work one evening shift per week
• May be required to work on Sat (once a month)
