DESIGNATION : Social Worker (PHBDC)

RESPONSIBILITIES
About the Job The Social Worker works with service clients and family in the Centre to provide social work-related support and assistance to their social, emotional and financial problems.
Job Responsibilities

• Work closely with clients and family with regards to their admissions and discharges.
• Assess client\xe2\x80\x99s support system/network and household condition as part of care planning.
• Work closely with multidisciplinary team and other healthcare professional to discuss care plans for total client\xe2\x80\x99s management.
• Liaise and make appropriate referrals to community service providers to meet the needs of the clients and their families.
• Maintain accurate and current documentation of casework.
• Conduct annual review of household financial information, financial stressors and to assist client and their families to apply for appropriate financial assistance.

QUALIFICATIONS
Degree/Graduate Diploma in Social Work Job Requirements
Minimum 3 years of experience in social service sector
Good interpersonal and communication skills with people of all levels
Ability to work independently
Demonstrate skill and knowledge in the eldercare/community care sector
