• Salary Up $4500 + AWS + Variable / Performance Bonuses

Job Responsibilities

• Screen applications and conduct interviews with clients and caregivers during enrollment into the programme or facility
• Conduct biopsychosocial assessments to evaluate the strengths, resilience, and social support systems of clients and their families
• Establish and maintain rapport with clients and their families, addressing their concerns, goals, and providing education on the physical and psychosocial needs of clients, as well as access to internal and external resources
• Provide casework support, coordinating with external agencies like hospitals and polyclinics for clients requiring financial assistance for medical appointments
• Develop and implement individualized care plans in collaboration with a multidisciplinary team (MDT) and provide therapeutic support as needed
• Oversee the execution of ICPs, monitor client progress, and develop structured programmes and activities aligned with organizational values to meet client care needs
• Coordinate with physical and occupational therapy departments to ensure clients\' comfort and confidence
• Prepare ICP review reports every six months, maintain monthly review records, and update client particulars and social reports. Conduct casework evaluations and maintain thorough case notes and documentation.
• Provide counseling services to clients and families, especially those experiencing poor mental health (e.g. depression, anxiety) or adjustment difficulties
• Assess risks of self-harm (e.g. suicide) or harm to others (e.g. family violence, elder abuse)
• Conduct financial assessments, manage client funds, and prepare periodic financial reports
• Refer cases for financial assistance and generate reports for subsidy changes
• Perform any other tasks as assigned by the management

Requirements

• Relevant qualifications in social work
• Diploma or degree in social work or relevant
• *SASW accredited professional
• Minimum 4 years of experience in social work
• Good knowledge of government financial schemes is a plus!
