Job Description

SAMH manages the Mobile Support Team (MST), it is a community based allied health-led team which provides psycho-social therapeutic interventions for clients with mental health needs and support for their caregivers to maintain clients wellbeing in the community.

Purpose

Responsible for the provision of case management services and support for persons in recovery from mental health conditions/possible mental health conditions, and their caregivers.

Core Duties:

Provide case management support and develop intervention plan Rehabilitative Services Outreach Involvements To conduct home visits when necessary Other administrative and support duties assigned

Qualifications

Degree in Psychology/Social Work/Counselling Some experience in case managment and mental health sector Self-motivated and self-directed Proactive Good Communication skills and resourceful Team spirit Experience in social service industry will be an advantage
