The H2H occupational therapist supports patients transition from hospital to home by providing comprehensive functional assessment, interventions, and caregiver training. The role focus on enabling safe discharge, promoting independence in daily activities, home modification for safer home, reducing hospital readmisssion and enhancing quality of life within the home and community environment.
The key responsibility includes
clinical care and assessment
Discharge and transitional planning
Community and home support
Care coordination and communication
