Description
- Perform care co-ordination for patients across the continuum of care from care facilities and into the community
- Perform assessment of patients\' needs, including functional and psycho-social needs, according to templates and help to link up with various resources and community services, based on patient\'s care plans
- Provide post-hospital discharge follow up calls to patients, according to protocols, to ensure resolution of their symptoms, optimisation of care and caregiver competency
- Handle customer enquiry or concern, by referring to call centre policies and procedures which are aligned with organizational standards, so complete, timely and accurate responses shall be provided
- Engaged, Informed Residents and Carers
- Provide education on chronic disease management and how to maintain healthy lifestyle. Then assess the ability of patient to manage own chronic conditions, guided by protocols
- Promote and encourage residents to participate in health and wellness programs that are available in their respective zones eg: Health screenings and community service partners
- Min Diploma in Biomedical Science
- Able to communicate effectively to incoming calls and enquiry
- Proficient in Computer navigation and administrative skills
- Able to perform rotating shifts, included weekend & Public Holiday
