Description
JD :
- Provision of case management on referrals assigned among the team. This includes intake accessment, identifying presenting issues, formulate care plans and make appropriate recommendations
- Delivery of social work service and programmes
- Conduct means testing, financial fee review and application of welfare programmes where applicable
- Conduct visit to different centres, home and hospital to provide supportive counselling
- Min Diploma in social work
- Some relevant experiences would be preferred but not compulsory
