Implement key strategies to shape and develop the community mental health ecosystem under the Community Mental Health Masterplan
Analyse service gaps and develop a new model of care to augment existing services in the community
Engage stakeholders to identify opportunities for collaboration and work with partners for capability building of the mental health ecosystem in accordance with the stepped care model competencies
Support regional engagement with the Regional Health Systems, restructured hospitals, polyclinics, GPs, and community partners to support population health objectives, strengthen health and social integration and streamline client referrals
Work with service providers on the timely submission of KPIs and claims for processing and reporting
Job Requirements
Degree in any discipline, preferably in Social Work / Psychology / Counselling with at least 2 years of relevant work experience in service planning, development, community services, or partners engagement
Experience in implementing new programmes, capability building, would be advantageous
Good team player and willing to embrace and manage change
Possess good written and verbal communication and presentation skills
Possess strong analytical and problem-solving skills with an eye for detail
