Job Description

• Plan and implement evidence-based wellness initiatives aimed at improving the quality of life of clients in the Community Care sector as well as those living in the community. These include meaningful recreational activities such as the arts (visual arts, creative movement), physical leisure (gardening, taichi), music, inter-generational projects, etc. Research, plan and implement evidence-based initiatives in collaboration with partners and the Community Care sector. Project manage, monitor and evaluate the outcomes and effectiveness of the initiatives. Engage and manage internal and external stakeholders to gain buy-in and partnerships. Partner with service providers closely in the processing and disbursement of approved grant funding. Review work processes for continuous improvement in service quality and grant governance. Perform all administrative and financial tasks required of the initiatives (writing approval papers, procurement of services, budget tracking, etc.). Job Requirements: Degree in any discipline with at least 2 years of working experience, preferably in healthcare, eldercare and/or social services. Self-motivated, resourceful with good problem-solving skills and a high level of initiative. Able to manage multiple projects with excellent project management skills and to thrive under pressure. Effective communicator (verbal, in writing) with good interpersonal skills and an interest to enhance care in the sector. Responsible, conscientious, detailed and a good team player. Candidates with prior programme management and evaluation experience preferred.
