Locations: West/North/Central/North-East
Working Hours: Mon-Fri & Half Sat, Clinic hours
:
- Be part of the care management team, coordinating and providing care activities to patients
- Perform basic preventive health screening and assessments when required
- Make appropriate referrals to care managers to change and improve lifestyle of patients
- Educate patients on home monitoring devices and using the HealthHub Portal or related mobile applications
- Document all care activities rendered to patients accurately
- Utilise IT systems to keep track of patient\'s appointments, lab tests and workshops and vaccination needs
- Collate data to facilitate Quality Improvement, Patient Safety and Research activities
- Assist in planning, organizing and coordinating health promotion activities in the workplace
- Other ad-hoc duties assigned
- Diploma in Health Management & Promotion or related fields
- Candidate who possess certification related to Health Promotion will have added advantage
- Preferably 1-2 years\' experience in patient fronting role
- Proficient in MS Office applications and navigating IT programs
- Good interpersonal and communication skills with passion in providing good service
