Challenges

Case management

  • Provide and coordinate continuity of inpatient and outpatient care for selected patients identified with high needs.
  • Act as pivotal team member for patient care by informing the multidisciplinary care management team of patient updates and initiate case review and discussion.
  • Perform the role of main liaison and contact person to monitor and collate referrals made by General Psychiatric services or other agencies to NAM
  • NAMS case manager roles also include providing continuity of care for patients in the community, he/she is expected to conduct site visits for brief intervention

Brokerage Role
  • Conduct assessment of patients for medical, psychological, social, environmental, vocational and financial needs, including monitoring of mental state examination, monitoring for signs of relapse and suicide risk.
  • Provide proactive problem-solving techniques in response to needs assessment, such as initiating appropriate referral and liaison with other professionals, agencies and/or step-down facilities.
  • Formulate and develop care management plans for patients in collaboration with clinicians and other health care members.
  • Monitor and evaluate patient’s response to treatment and revising treatment/care plan as needed.
  • Establish a thorough working knowledge of resources and information for dissemination to patient and family.

Therapeutic Role
  • Establish rapport and build therapeutic alliance with patient and family.
  • Provide motivational and supportive therapy for patient and family in response to their needs
  • Provide psychoeducation to patient and family.
  • Telephonic case management within 48 hours of discharge
  • TCU reminder prior to appointment
  • Home visits for Alcohol Related Frequent Attenders (ARFA)

Requirements
  • Degree in Psychology, Counselling, Social Work, Nursing or related discipline with relevant work experience
  • Basic knowledge of PC applications (Microsoft Words & Excel)
  • Excellent interpersonal and written communication skills
  • Individual needs to be self-directed, with excellent social skills, and able to network with a wide range of professionals
  • Experience in working with clients in the community setting is an advantage
