Broad Function

The Manager must be able to:

i. Lead and drive resident activation and partnerships, with internal and external stakeholders, for better health for residents as a key Regional Health Office\'s member;

ii. Lead, develop and evaluate community outreach programmes to improve health literacy, build awareness on preventive health, and inculcate right health seeking behaviour; and

iii. Participate actively in the development and implementation of integrated care programs within and between the different care settings internally within the Organanisation and relevant teams in NHG and externally with partners in the community across various health and social settings.

Key Responsibilities and Duties

Specific

A. Partnerships

1. Develop a partnership strategy and implement it for a strong Heathcare Network.

2. Build and sustain community partnerships in areas of co-creating a community of practice in driving community activation.

3. Create and build a vibrant community with sharing of experiences, best practices and exchange of ideas.

4. Conceptualise and organise regular partnership leadership, co-creation and networking platforms for multiple stakeholders.

5. Lead the team in organising and participating in various community engagements and events

6. Develop an internal account management framework and tools to support colleagues.

7. Optimise the use of NHG CRM to manage partnerships and conversations.

B. Resident Activation

1. Develop a resident activation strategy and model, and implement it through various channels internally and externally.

2. Develop and implement a framework and environment for residents to take charge of their own health and to activate them to care for others in the community

3. Identify appropriate and validated tools to enable residents to assess their activation levels and build resources, knowledge, and support to increase their activation levels.

4. Lead teams and execute large scale events within set budgets and timeline.

C. Community Programmes Development & Implementation

1. Develop a strategy to inculcate good health behaviors for schools, workplaces, and the community. This can include health and wellness, community screening, health coaching and literacy, caregiver support and relevant programmes and services.

2. Work with clinical teams and community partners to develop the programmes, project manage and implement them through an operational model.

3. Develop a strong and deep understanding of the target profiles, and develop evaluation and measurement tools.

4. Synergise with agencies and community partners to systematically deliver good health outcomes for residents, including children and youths in schools.

5. Continuously review and improve on programmes for schools, workplaces and the community.

6. Specific for Organization\'s School Health & Wellness programme, to deliver selected curriculum to the teachers and partners

D. Regional Health Office secretariat

1. Be part of Regional Health Office secretariat to drive the agenda for Organisation\'s Integrated Care Network (ICN).

2. Organise meetings and drive discussions with Woodlands Health ICO Ex-Co.

General

The Manager must take on new assignments and projects with enthusiasm as and when needed.

JOB REQUIREMENTS

  • Recognised University Degree, preferably in health and social sciences or business degrees
  • Minimum 8 years of relevant experience in healthcare, community, or social care sector
  • Experience in programme development & delivery, training, and partnerships
  • Proficiency in Microsoft Office products and business analytics
  • Strong project management skills, with the ability to facilitate discussions and translate concepts into frameworks and care and operational models
  • Strong process and systems thinker, with focus internal/external user experience
  • Self-motivated, driven, and able to work well with multiple stakeholders
  • Create problem solver with strong interpersonal and communication skills
  • Good analytical, presentation and writing skills
  • Passion in transforming public healthcare and serving the community
  • Ability to organize, plan, lead and navigate complex systems
  • Prior supervisory experience will be good to have.
  • Weekend work can be expected for community engagements.
