Job Description

Group Integrated Care (GIC) drives and supports the strategic development and implementation of NHG's Population Health Strategy, in synchronisation with the MOH Healthier SG (HSG) strategy. GIC achieves this through driving and coordinating integrat Group Integrated Care (GIC) drives and supports the strategic development and implementation of NHG's Population Health Strategy, in synchronisation with the MOH Healthier SG (HSG) strategy. GIC achieves this through driving and coordinating integrated care planning and programmes, new clinical capabilities, finance and performance transformation, partnerships and engagement, and key enablers such as digital enablement and workforce transformation. It also supports the development and operations of NHG Cares and NHG's Integrated Care Organisations (ICOs) serving their respective population zones (Central, Yishun and Woodlands). The Manager of NHG GIC Operations (Planning) will support the Assistant Director and Director in driving NHG population health and HSG development, coordination and implementation, with a focus on strategic planning, digital and data efforts in NHG's transformation to a population health system. Key activities will include but not limited to (1) to manage strategic planning activities and support NHG management in the development of population health strategy, framework and plans, (2) to provide secretariat support for key management platforms, (3) to develop and plan NHG implementation of HSG in coordination with the ICOs, particularly for HSG operations and enablers at the cluster level, in alignment MOH and key partners, and (4) to assist in developing a viable digital and data business strategy and implement its products. In the course of work, he or she will also work closely with the Integrated Care Organisations (ICO) and NHG Cares under the National Healthcare Group (NHG) to ensure smooth and effective engagement and implementation. Major Duties & Responsibilities Population Health Strategic Planning and Management Platforms To manage strategic planning activities in support of NHG management in the development of population health strategy, framework, and plans. To liaise and coordinate on behalf of NHG management with MOH and key internal and external stakeholders on population health matters. To provide secretariat support for key management platforms. This includes setting up of the committee, development of appointment letters, minutes taking, coordination of meeting agenda, preparation of presentation materials and follow ups. Healthier SG (HSG) Implementation To develop and plan NHG implementation of HSG in coordination with the ICOs, particularly for HSG operations and enablers at the cluster level, in alignment MOH and key partners. To function as a key point of contact between MOH and NHG on matters relating to HSG, and across the ICOs and relevant NHG units. Digital and Data Transformation To assist in conceptualising/ updating a viable digital and data business strategy that supports NHG Population Health efforts. To gather, align and integrate the business requirements and use cases while balancing the interests of diverse users and stakeholders for both digital solutions and data needs. To scope and develop digital products which are essential for business requirements and achievable by the NHG Population Health milestones timeline. To review and secure access to data required for population health. Undertaking any other tasks and duties as assigned by the Supervisor or Head of Department. Job Requirements Education/Experience Good bachelor degree in any discipline. Preferably with 5 years of relevant working experience in Healthcare sector and/or strong background in the areas of IT and strategic planning. Demonstrated leadership, teamwork, abilities in managing internal/external stakeholders with diverse interest in a dynamic environment. Experience in delivering projects of similar nature across the full life cycle. Must possess strong forward planning, strategic thinking, administrative and project coordination skills. Strong written, analytical and communication skills. Proficient in various computer applications, including Microsoft Word, Excel & PowerPoint System-thinker with problem-solving skills in a dynamic and unstructured work environment. Good interpersonal & communications skills and ability to manage stakeholders from diverse backgrounds at various levels of seniority. Personal Attributes Mature in disposition in relation to personal character. Strong self-discipline, sense of responsibility & determination. Ability to work independently. Self-motivated and result-oriented. Resourceful and able to work in ambiguous situation with a can-do attitude. Strong team player that is team & organisation focused, with the ability to build and maintain harmonious working relationships with internal/external stakeholders. Attention to detail and able to multi-task and manage various project elements and timelines. Demonstrate ability in leading and nurturing peers.
