Develop referralandcare processes to enable the flowandescalation of clients in primary care to support chronic physicalmentalhealth conditions. Conduct ongoing review to developnewmodel of care in delivery of joint management formentalhealthandchronic condition services in the primary care setting focusing on GP. Developworkinstruction fornewservices including continuous reviewandenhancement of the care models to meet the intended outcomesandsupport the needs of the clientsandcaregivers. Managementalhealth capability building strategy in primary care sector. Formulateandimplement plans to strengthen the capability of primary care providers focusing on GP, in providing the required training in accordance to the stepped care model competencies. Support emerging needsandcontinuous upskilling to meet the needs of clients. Establish a sustainable financingandresourcing framework. Responsible for the overall programmes' key performance indicators, including the managementandutilisation of the allocated budget. Prepare annual budgetandprogramme progress updates. Analyse programme care modelandto propose funding model at steady state. Manageandlead a group of programme managers. Provide supervision, guidanceandadvice to assigned team members. Develop platforms for discussionandcross-learning with staff on programme management. JOB REQUIREMENTS BachelorDegree/PostGraduateDiploma/ProfessionalDegree. At least 8years' experience, with at least 2yearsin a managerial capacity,andfamiliar with multi-agencywork/healthcare industry, programme implementationand/orprogramme management. Experienceandknowledge in facilitation, design thinking, organization development preferred. Experience in primary careorcommunity developmentandmulti-agencyworkin healthcare/social care industry would be an advantage. Able toworkwell with groupsfromdiverse backgrounds. Able to multi-taskandpossessstronganalytical, problem solvingandproject management skills. A self-starter withstronglearning ability, who is resilient, adaptableandable to manage changeandambiguity. Possessstrongnegotiationandcommunication skills.
