To implement mental health in primary care programmes and improve the collaborative partnership within the ecosystem to achieve seamless process and structure in identification and referral of clients.
Drive implementation of Healthier SG focusing on mental health (MH), PCN MH,
MH GP Partnership, and covering Health and Mind, and Dementia services. * Develop referral and care processes to enable the flow and escalation of clients in primary care to support chronic physical mental health conditions.
Conduct ongoing review to develop new model of care in delivery of joint management for mental health and chronic condition services in the primary care setting focusing on GP.
Develop work instruction for new services including continuous review and enhancement of the care models to meet the intended outcomes and support the needs of the clients and caregivers.
Manage mental health capability building strategy in primary care sector.
Formulate and implement plans to strengthen the capability of primary care providers focusing on GP, in providing the required training in accordance to the stepped care model competencies.
Support emerging needs and continuous upskilling to meet the needs of clients.
Establish a sustainable financing and resourcing framework.
Responsible for the overall programmes\xe2\x80\x99 key performance indicators, including the management and utilisation of the allocated budget.
Prepare annual budget and programme progress updates.
Analyse programme care model and to propose funding model at steady state.
Manage and lead a group of programme managers.
Provide supervision, guidance and advice to assigned team members.
Develop platforms for discussion and cross-learning with staff on programme management.
At least 8 years\xe2\x80\x99 experience, with at least 2 years in a managerial capacity, and familiar with multi-agency work/healthcare industry, programme implementation and/or programme management.
Experience and knowledge in facilitation, design thinking, organization development preferred.
Experience in primary care or community development and multi-agency work in healthcare/social care industry would be an advantage.
Able to work well with groups from diverse backgrounds.
Able to multi-task and possess strong analytical, problem solving and project management skills.
A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity.
Possess strong negotiation and communication skills.
