Engage the beneficiaries through individual or group-based preventive activities and progra Primary Objectives of Position Organize & conduct community outreach events and activities to raise awareness of mental health, including dementia Engage the beneficiaries through individual or group-based preventive activities and programmes, through home visits or at community site(s), targeting their holistic health and well-being to promote mental wellness and social connectivity Support & monitor clients' & caregivers' condition and progress Job Responsibilities Coordinate & support outreach events and provide logistical & operational support to ensure the smooth organisation of these events Promote, coordinate and conduct a variety of meaningful health & social activities to consistently engage the community Organize & conduct structured preventive activities and/or programmes for the community, those at risk of mental health conditions and caregivers Provide basic mental health information to seniors & caregivers Monitor clients' conditions and progress by conducting home visits or follow-up calls, engage clients in meaningful activities based on client's interest, and escalate cases for support as necessary Facilitate communications between the programme and client/family/caregivers and/or the partners Document clients' progress & track client's participation in outreach & activities and assist in collation of statistics for submission to stakeholders Perform administrative duties for programme / department, ensure client's data is updated on a monthly basis, maintain centre's expenditure and coordinating the team training schedule etc Upkeep & maintain accurate client records, statistics, programme data and documents, in accordance to programme requirement, stakeholders reporting and prevailing personal data protection security guidelines Extend support by contributing in different projects and committees from time to time, as and when required Technical Competencies: - Minimum GCE 'O' levels, up to Diploma graduates - IT literate Preferred characteristics: - Proactive, energetic & passionate to serve the community - Team player, yet independent to carry out most functions - Ability to speak dialects will be advantageous
