Managing telephone/email enquiries on Home Care services, and referrals from external agencies and internal sources pertaining to out scope of services and related charges
Processing new referrals from relevant platforms/systems, in accordance to department\xe2\x80\x99s standard operating procedure (SOP)
Coordinating with the Nursing Team for new clients\xe2\x80\x99 initial assessments, periodic medical/social assessment and update the approved care plan into the related files/systems/database
Planning service schedules and disseminating to the assigned care staff, and briefing care staff on the specific scope of services to be rendered for each assigned client
Close coordination with Finance Team to ensure submissions of staff\xe2\x80\x99s remittances are accurate with all necessary adjustments on work schedule reflected in the relevant files
