The incumbent will assist in implementing and supporting Population Health initiatives in the Western region. He/she will contribute towards the nationwide implementation of Healthier SG by working with partners and stakeholders, both externally and internally.
Duties and responsibilities
- Drive Population Health intiatives in primary care
- Engage and form collaborative relationships with internal and external partners and stakeholders as required
- Implement the performance evaluation matrix and oversee the analysis of data
- Administer and manage Population Health-related finances and audit
- Co-drive and provide administrative support to Quality Improvement projects
- Provide secretariat support for key committees and workgroups, and gather insights/ feedback from partners for the planning and implementation of Population Health initiatives
- Any other tasks as assigned
Job Requirements
- Tertiary Education, preferably in health or social related fields
- Minimum 3 years of healthcare-related experience. Prior experience working with doctors will be an advantage
- Possess an analytical and inquisitive mind for problem-solving, and the ability to identify/ discern complex trends and patterns. Prior experience in Quality Improvement will be an advantage
- Highly motivated, independent and a good team player
- Good interpersonal, communication and negotiation skills
- Ability to multi-task
- Excellent writing skills
- Proficiency in Excel to consolidate/ cross-check data and perform sumple analysis & chart results. Experience working with business intelligence tools (e.g. Power BI, Tableau) will be an advantage
